Notes:

(1) All contract changes and/or task orders approved by the general manager shall be promptly reported to the board.

(2) All contract changes and/or task orders shall be approved by the general manager or board prior to payment.

(3) The decision threshold amounts above should not include sales tax. However, the final cost of all task orders and change orders reported to, or approved by, the board shall include the appropriate sales tax amount.

(4) For efficiency, individual change order items may be combined into a single change order for approval by the board.

(5) All change orders shall address changes to contract working time, or state if no change.

(6) Markup rates on change orders shall be in accordance with the contract documents.

(7) Unless there are extenuating circumstances, all stakeholders must sign the change order for full execution, including the district, contractor, and consultant if applicable. [Res. 860 § 1 (Exh. 1), 2024.]
