The district shall mail utility bills to the mailing address provided by customer for water and sewer service provided by the district. The district’s utility bill shall contain the following information:

(1) The account’s service address or location;

(2) The name of the customer;

(3) The customer’s account number;

(4) The dates of service;

(5) The bill date;

(6) The due date;

(7) The outstanding balance prior to the bill;

(8) The current charges;
